New FBC Keller Release of Liability Waver and Medical Release
Wehave changed our Release of Liability and Medical Release Forms to better suit everyone's needs. We now have two parts to the form. The first part is the Release of Liability agreement. It gives permission for your student to attend the event and releases FBC Keller from any liability from property damage or injury of the student. This section must be completed and notorized for each individual event. The second part is for all medical information and is kept on file for one year.
